The common ones are well-known but still routinely ignored — fuzzy ROI targets, employees left out of the change process, and leaders who approve budgets without understanding what they're approving.

The unexpected ones are where it gets interesting:

  1. Shadow AI is the sleeper issue — when employees can't get official tools approved, they use ChatGPT or Claude on personal accounts. The company thinks it has an AI strategy; it actually has an unmonitored patchwork.
  2. Automation bias creeps in silently. Once a team trusts the model, they stop reading the output critically. Errors compound downstream before anyone notices.
    Pilot purgatory is extremely common in large enterprises — a proof-of-concept succeeds, everyone celebrates, and then it sits in limbo for 18 months because no one owns the productionization step.
  3. Prompt brittleness catches companies off guard: workflows get built around specific model behaviors, a vendor updates the model, and suddenly customer-facing processes break in subtle ways.
  4. Political sabotage rarely gets named openly. Middle managers whose value comes from information control or manual processes have strong incentives to quietly slow-walk adoption. It looks like implementation delays; it's actually survival instincts.

The Pattern Every Company Goes Through

AI adoption follows a familiar trajectory.

AI Doesn’t Create Chaos — It Exposes It

This is the part leaders often miss.
If your workflows are already disciplined, AI helps them move faster.
If your workflows are vague, AI amplifies that vagueness.
It exposes:

  1. weak prioritization
  2. unclear ownership
  3. inconsistent standards

That is why two firms can buy access to the same frontier models and end up with very different outcomes. The technology is similar. The management system around it is not.

Microsoft’s 2025 Work Trend Index points in the same direction. Based on survey data from 31,000 workers across 31 countries, LinkedIn labor-market data, and Microsoft 365 productivity signals, the company argues that this is a pivotal year for redesigning how work is organized, not just which tools people use. In fact, 82% of leaders said 2025 is a pivotal year to rethink core aspects of strategy and operations.

That is not a tool's problem. That is a leadership problem.

The companies getting real value are doing something different

The common assumption is that AI adoption is about experimentation. To a point, that is true. But the companies creating durable value are not the ones running the most pilots. They are the ones turning isolated experiments into repeatable systems.

McKinsey’s latest research found that organizations seeing higher value from AI are more likely to have senior leadership ownership, clear governance, human validation processes, and management practices that connect AI use to strategy, talent, technology, data, adoption, and scaling.

In other words, they don’t ask, “Where can we play with AI?”

They ask, “Where does AI belong in the business, and what changes because of that?”
That is a very different question.

GitHub’s own enterprise research tells a similar story from the engineering side. Their 2025 enterprise Octoverse report says AI coding tools are now central to software development, with reported productivity up 55% and code quality improving when the tools are used effectively. But the important phrase there is “used effectively.” Those gains don’t come from buying licenses. They come from changing how teams work.

What leaders might have done wrong

There are key mistakes I see again and again as a CEO.

1. Start with the tool, not the bottleneck
“Let’s use AI in engineering.” “Let’s add AI to customer support.” “Let’s automate knowledge work.”

Those are not strategies. They are vague ambitions.

Better questions are:

  1. Where are we losing time, consistency, or judgment today?
  2. If the answer is code review, use AI there.
  3. If the answer is onboarding, use AI there.
  4. If the answer is incident summarization, use AI there.
  5. AI becomes useful when it is attached to friction.

2. Diffuse ownership

AI often lands in a strange no-man’s-land. Innovation owns the pilot. IT owns the platform. Business units own use cases. Nobody owns outcomes.

This is one reason scaling stalls. McKinsey found senior leadership ownership is one of the factors that most clearly distinguishes AI high performers from everyone else.
If AI is everyone’s side project, it becomes no one’s responsibility.

3. Celebrate activity instead of value

Many companies mistake usage for progress. People log in. Prompts are written. Content gets generated. Code is suggested. Dashboards light up.

But where is the actual business effect?

Deloitte’s most recent enterprise AI reporting shows that leaders are now turning the corner from experimentation to scale and asking harder questions: ROI, ethical practices, workforce readiness, and concrete go-to-market moves. Their 2026 survey edition covered 3,235 business and IT leaders across 24 countries, which tells you how broad and mainstream this question has become.
The lesson is simple: activity is not value.

4. They leave the operating model untouched.
This is the biggest one. AI changes decision speed. It changes how knowledge is accessed, who can do work, and how quickly bad habits scale.

If you deploy AI without changing workflows, governance, and accountability, you are just pouring jet fuel into the old system.

That can look like progress for a quarter. Then the friction shows up. The future belongs to redesigned systems, not enthusiastic pilots

The phrase matters because it shifts the frame. AI is not just a software feature. It is part of workforce design.

The same Microsoft research argues that organizations are moving toward what it calls “Frontier Firms,” where humans and AI agents are assembled more deliberately around roles, functions, and projects.
That is where many companies still fall short. They buy AI, but they don’t redesign work around it.

And without that redesign, adoption gets stuck in the middle:

  • too widespread to be called a pilot,
  • too unstructured to be called a strategy.

What to do instead

The fix is not complicated. It is just harder than buying seats.
First, pick the bottlenecks that matter.  Not ten. Two or three.
Second, assign clear ownership.
Not “shared accountability.” One accountable executive. Third, define what success looks like in operational terms:

  • cycle time
  • error rate
  • conversion improvement
  • support deflection
  • onboarding speed
  • margin expansion


Fourth, redesign the workflow, not just the task.
Where does human review stay?
Where is AI allowed to suggest?
What gets automated?
What gets escalated?
Fifth, train managers, not just employees.

This is where many programs quietly fail. Employees are often more ready than leaders. McKinsey says exactly that: people are more prepared to use AI than leaders are prepared to steer it. That matters because AI adoption is not just a capability issue. It is a management issue.

Uncomfortable truth

By 2026, “we use AI” will no longer be a differentiator. GitHub’s 2025 Octoverse shows developer activity reaching record highs, with monthly average pull requests merged rising from 35 million to 43.2 million, and code pushes from 65 million to 82.19 million. AI is already changing the scale and speed of software work.

The differentiator will be whether a company has built a coherent system around that new speed.

No more tools.
No more pilots.
No more AI implementation.

Just a clearer answer to a very old management question:

What problem are we solving, and how will we know if this is actually working?

That is where strategy begins.
And that is exactly where most AI chaos could have been avoided.

A better question to leave with

Instead of asking, “How fast can we adopt AI?” try asking:
Which workflow should be redesigned first because of AI?

  1. Who is accountable for business outcomes, not just experimentation?
  2. What would need to be true for us to call this successful six months from now?
  3. Are we building an AI program, or are we redesigning the company?


Most companies won’t fail at AI because the models disappoint them. They’ll fail because they never made the management decisions that turn a tool into a system.
